For a few hours on Friday morning, Pakistan's most famous inmate was no longer in prison.
Then he was.
Former Pakistan prime minister Imran Khan was moved out of Rawalpindi's Adiala Jail for a medical examination following a Supreme Court order, only to be returned to custody shortly afterwards. Doctors declared him medically fit. His family and political party said the authorities failed to fully comply with the court's instructions. The government insisted it had acted appropriately.

Earlier this week, Pakistan's Supreme Court ordered authorities to transfer Khan from Adiala Jail to Islamabad's Shifa International Hospital after hearing petitions from his family and Pakistan Tehreek-e-Insaf, which had raised concerns about his health. The court also directed that a medical board examine him and that his personal physician be associated with the process.
The judges said the order was necessary to protect Khan's constitutional right to health and medical care. Reuters reported that the court specifically cited concerns raised by his family and lawyers, who have repeatedly argued that his condition has deteriorated during his imprisonment.
What Are The Health Concerns Surrounding Imran Khan?
This is where the dispute begins.
Khan's family, lawyers and PTI leaders have for months argued that the 73-year-old former prime minister is suffering from a range of medical problems, including significant vision issues in his right eye. His sons have publicly expressed concern about his health, while his legal team has repeatedly sought specialist treatment outside prison.
Court documents submitted by jail authorities paint a different picture.
According to reports presented to the Supreme Court, Khan has undergone numerous medical examinations while in custody. Jail officials told the court that his vision had improved significantly after treatment and was now "almost normal". The same reports said he receives regular medical monitoring inside prison.

Khan has experienced fluctuations in blood pressure and anxiety. A report submitted by prison authorities reportedly recommended greater interaction with family members to help address stress-related concerns.
Why Is The Hospital Visit Controversial?
Because the argument is no longer only about health.
It is also about whether authorities followed the court's instructions.
According to Reuters and the Associated Press, the Supreme Court had directed that Khan be transferred to Shifa International Hospital. However, his family later alleged that he was instead examined at the government-run Pakistan Institute of Medical Sciences (PIMS) before being returned to jail. They argue this violated the spirit, and possibly the letter, of the court's order.
The government says security concerns forced the change.
Information Minister Attaullah Tarar said authorities chose PIMS because of security issues around the route to the private hospital and the presence of PTI supporters near the facility. He added that medical specialists examined Khan and found no urgent reason for hospitalisation.
PTI has indicated it may pursue further legal action over the issue.
Why Does Imran Khan Remain Such A Powerful Political Figure?
Even from a prison cell, Khan remains the central figure in Pakistani politics.
The former cricket captain led Pakistan to its only Cricket World Cup title in 1992 before entering politics and becoming prime minister in 2018. He was removed from office through a no-confidence vote in April 2022 and has been jailed since August 2023 in a series of cases that he and his party describe as politically motivated. The government rejects that allegation.
Despite his imprisonment, PTI continues to command significant public support, and every development involving Khan—whether legal, political or medical—quickly becomes national news.

The hospital visit was brief.
The debate it triggered is unlikely to be.
For Khan's supporters, Friday's events reinforce long-standing concerns about his treatment in custody. For the government, the medical examination showed that he is receiving appropriate care and does not require extended hospitalisation.

Imran Khan is back in Adiala Jail.
The larger questions—about his health, his future and Pakistan's deeply polarised politics—remain very much outside the prison walls.
